<p>I got this one correct, but I just want to clarify something. It's not grammatically correct EVER to say "had ran" or "had swam" - it should always be "had run" and "had swum" when "had" is used right?</p>

<p>ran is past tense, the past participle for run is run, so in this case it's "had run".
