1099 income: once more

My D had income last year reported on a 1099misc (from a summer internship). Our tax preparer has told us she needs to pay social security on this amount and that to do so she must:

File a 1040.

Can enter the amount on line 21 of the 1040. She does not need to file a schedule C as she had no expenses associated with this income.

That she will need to pay $600 in SS.

Our questions: does she need to include another form? I know that SS is about 15% of that income, but is there a chart she gets the total tax from? We looked at Form SE, but it asks for income from another form. So how do we show on the 1040 where she gets the total amoutn of SS she has to pay?

PS she also had a very small amount of income (wages/W2) from jobs at her school.

report the income on Form C-EX; carry the total to Form SE and calculate the tax, which is then reported on line 56 of 1040 Form

Thanks for the information!
