<p>I just sent in the rest of my app to the afa, and a couple days after i realized that i made a mistake on the drug abuse form. Apparently you must initial ALL of the columns…I only initialed the last 4-5 ones before having my high school prinicipal sign it. I just made checks for the first four ones (one yes and all the rest in no column)…so…I’m guessing that I may have to send in a new one? anyone have a similiar experience with their app? ahhh what a stupid mistake…</p>
<p>I accidentally signed the back of mine, which you are only supposed to do on inprocessing day.</p>
<p>Worse comes to worse, they will let you know and you can have to fill out another one. I think the form is actually available online (if you loginto your status page you can download it if you havent turned it in yet), fill it out again then sent it in.</p>

<p>If that's the form I'm thinking of (concerning whether or not you've ever done any kind of drug, etc.), we filled out another one on inprocessing day. It may have changed in the past 4 years, but we did another one from scratch to give everyone a second chance to answer honestly, in case people were unwilling to answer it truthfully initially (due to parents looking over all the paperwork or whatever).</p>

<p>Actually the first year i left the whole thing on and whited it out. This year i was smart and didnt. But i think i made a mark on the back which to anyone else would be disregarded, but i promised myself id see it again.</p>
<p>They will reject the one where you did not initial all of the boxes. You can download another one, have it filled out, and sent it in again. It might save some time.</p>

<p>My ALO instructed me to sign the back and my admissions counselor had no problem with this and since I already have accepted my appointment should I have any trouble later on?</p>
