addressing letter of recommendation

I will give you a scenario. If someone has a recommendation but the rec came from a teacher from a different school, how should that person address the envelope? I'm sure the specific college's admissions address will go in the middle. How about the return address? Should that person write his own address or the address of the other school? How about names? Anything else that needs to go on the envelope? If you only received one genuine rec, can you copy and print multiple copies of it to send to additional schools?

Thanks to anyone who answers some or all of these questions. Any comment or suggestion is also helpful. This scenario is to help those who are confused about this matter (including myself, obviously...that's why i posted this thread :D).

You can copy and print multiple copies if you get the permission of the writer.

Rec letters should be addressed to applicable admissions office of the college and return address should be that of the person who sends it (other school if that is where he is or alternatively the sender’s home address). A return address is provided in the event the envelope cannot be delivered (usually because someone put the wrong address for the recipient college) and is returned. If you give a return address other than the sender and it gets returned to someone other than the sender, it is likely to end up in someone’s garbage can. Nothing other than the proper address of the admission office needs to go on the envelope. Other than recs submitted through common app procedure, any rec should be an original letter signed by the sender and not a photocopy.

I’m a bit confused on the photocopies, since I read in some places that photocopies of recommendation letters are allowed.
I’m also confused that if there is no additional information on the envelope, how would the admissions office distinguish the name which got recommended in the letter from names that are identical?
