My ACT math score is awful. The first time I ever took a practice test (last Septemeber) I got a 22 math. I studied and got more familiar with the types of ACT problems and was able to get a 27 on my real ACT math in December. But that's the best I've ever gotten and usually (even now) get 25s or 26s on practice tests. It's just so disappointing to me though because my English and Reading are always 35s and 36s, and my Science is usually at least 30 also.

So basically there's a 10 point difference between my English&Reading scores, and my Math score. I feel like its hopeless and ill get my math score up to par, so has anyone actually improved their math (or any subsection) score by 5+ points? Please let me know its possible!!

It is most definitely possible! When I started with the reading, I was getting 24s and 25s while English and math were floating around in the high 20s and 30s (science is a different story). I got tutored in it and it was extremely helpful. If you can’t afford a tutor or don’t have the time for one, just practice a ton and learn what you do wrong. With math, just know what things you need to look over (trig, geometry, etc) and practice them a lot. Keep in mind that some of the questions on the math, especially towards the end, are extremely tricky.
My reading score on the last test I took ended up being a 32 (on the first real test I took it was a 26). My English score also jumped from a 28 to a 35, so it most definitely is possible, it just takes practice! It won’t just happen overnight.
