Applying to Cal Poly SLO and Cal Poly Pomona?

<p>So I'm applying to both Cal Poly SLO and Cal Poly Pomona and was wondering if I have to fill out two forms and then submit it? Is it like the UC's where you fill out one form and it shared through out the campuses? Is this also for the CSU?</p>
