Yes, you have to be admitted to the school, so fill out an application. Sometimes the coaches submit an outline of the full application, and sometimes they will waive the application fee. Usually the coaches don’t have all the information to fill out the entire application, and don’t have official transcripts or scores. If the athletic scholarship is not a full scholarship, you may qualify for other merit aid through the regular application process.
Admissions office makes the final decisions on recruits, not the coaches. It’s important to follow every step of the application process unless specifically directed otherwise. Most coaches will have you fill out some basic academic stats for a pre-read prior to starting the process.

I have known of athletes that were offered scholarships but unable to get into the school, so it’s not a slam dunk
Not a dumb question it varies by school, sport and recruit.
Ask the coach if there is any code to include, some schools have a place on the application for an athletic recruit number. Also depending on the timing, you may need to be issued a sign in to access the application if the normal submission timing has passed. The coach can also tell you if the personal statement essay is required, or if it can be left blank.
You will always need to submit an application, but the process and requirements for what is considered complete can vary significantly. If you are not explicitly told otherwise, assume you have it fill it out completely and wait for an admissions decision. Not getting admitted will void the NLI, so recruiting is not done until the admission letter is in hand
