Behind the scenes

I've been in college for a year and there are still 2 things that I haven't figured out.
What exactly happens at the end of the semester when the "school's committee" meets? Do they talk about each student particularly, check out their grades and write comments down somewhere? Do they see any other grades than the final grades? Do they only talk about people who have problems? Do professors get to say something about the student?
I guess what I'm asking is what is behind that transcript that we get at the end of the year and how is our performance evaluated.

I've kind of the same question for college applications (and esp transfers). What happens once you hit the "submit" button? Who reads what, in which order? Do they print out the stuff and really have two piles "accept"/"reject"? Can you file be thrown by some admission officer who didn't like you and nobody ever knows about it or do they have multiple people reading them? Do they read everything or kinda stop once they have made up their minds (ie they read your essay, it sucks, the end)?

Lastly, what does a 3.6 gpa say about a person and what's the gap between 3.6 and 4.0 (how many more studying hours)?
There is no motive to these questions. I'm just being curious and random and also I can't sleep.
