Born outside of the US

I am applying to West Point this year and remember looking through the application and seeing something that said that if I was born outside of the US, I had to fill out something similar to this form. Both my parents are natural us citizens and they moved to England where they had me. I recieved US citizenship at birth from the embassey in London. Do I have to fill out this form and if so, can anyone point me to it? Thanks for the help.

No, I do not believe there is a form for candidates to fill out while they are in the application process. However, once a candidate accepts an offer of appointment, he/she will need to prove citizenship by submitting a certified copy of his/her birth certificate or naturalization certificate.

As an update, things have changed with the new online application. When you fill out your CPDR (Candidate Personal Data Record) you are instructed to submit State Form 240 if you were born outside of the US to US citizens.

“The official record is in the form of a Consular Report of Birth Abroad of a Citizen of the United States of America. This document, referred to as the Consular Report of Birth or FS-240, is considered a basic United States citizenship document. An original FS-240 is furnished to the parent(s) at the time the registration is approved.”
