Can I transfer from a community college to a conservatory??

This has really been troubling me for the past few weeks now.
Currently, I am a gap year student. I am beginning school at a community college this upcoming Fall, but I'd eventually like to transfer to a top conservatory like Colburn.

If someone can give me some honest advice, I would be more than greatful! :)

I was not academically strong in high school, but I am passionate about my music. I am 100% sure that this is what I want to do for the rest of my life.
Because I never took high school seriously, I made the tough decision of taking this year completely off and begin my higher education this upcoming Fall at a community college.

I am doing very well with my music this year. I just finished learning the Bruch Violin Concerto in G minor and Czardas. Right now, I am working on the Mendelssohn Violin Concerto in E minor.
This is also my second year in a youth symphony and we are working on Beethoven, Wagner, Berlioz, Franck, Mozart, and Vivaldi.
So, musically, I think I am making steady progress.

My question is, can I go to a two year college (with the mindset of going to a conservatory) and then transfer into Colburn School of Music or a similar conservatory?

Thank you very much! I hope I can find some useful advice here! I really have the desire to pursue music, but the path that I have to take is what I am uncertain with.
