Chances with one bad grade?

I am a 4.0 UW GPA student (5.17 W) who has taken full IB throughout high school (I am a senior now) with fairly strong test scores (33 ACT). However, I am very worried due to the fact that I am currently struggling quite a bit in my IB 20th Century History Class. Although I currently have A's in all of my other classes, I am borderline C/D right now for history. My teacher uses intimidation methods both in class and through her grading to keep control of her students, so even the strongest history students have middle-range B's. I am guessing that I will most likely end up with a C or a B if I am lucky.

I am now applying to a handful of schools, but am worried about admissions chances being reduced by this one major blemish on my record. My top choices include UNC (I am in-state), Duke, and Vanderbilt. Obviously this situation is not ideal, so I am wondering if getting one C at mid-year reports will ruin my chances at admission. Should I be adding a few extra backup colleges to my list of schools I am applying to?
