Dependent Student Verification Form

I’ve been selected to supply information for the Dependent Student Verification Form, but am confused on what I should put for myself under “College Name” and “Enrolled at Least Half-Time.” I know I will be enrolled in college, but what am I supposed to put for “College Name”? Do they want me to put OSU, or should I just leave it blank?

Since this form is just going to OSU, I think you can just put that in for yourself…I assume you haven’t accepted their offer of admissions - right?

Yes, I haven’t even been accepted yet… which makes me feel a bit awkward putting OSU in. But I suppose there is no harm in doing so. Thank you for your insight
