Didn't sign waiver

A few weeks ago, I gave the Common Application's "College Instructor Evaluation" form to 2 former professors to send out to the colleges I am applying for transfer to.

However, I have just now realized that I forgot to fill in the red box on the "College Instructor Evaluation" form for one of my professors because I had emailed him a PDF of the form. The box requires the student to choose whether or not to waive his right to access and also requires a signature. This professor has already sent the evaluations out.

Am I completely screwed now? What could I possibly do to fix this? I am applying to Cornell, Georgetown and a number of other top colleges. My grades and SATs are excellent but I'm kind of nervous that this tiny thing might ruin my chances.

Call and ask?

I really don’t think this is a big deal.

Don’t expend any more energy worrying about this one. Some people waive their rights, some people don’t, some people flat out forget to decide, and a responsible admissions officer is not going to second guess why you did what you did. Even if they wanted to, they won’t have time to because they have to read a bazillion LORs each year!

If they believe you are a good candidate, your referees will write you solid LORs whether or not you waive your rights. That is what ultimately matters.
