<p>I like arcadia’s suggestion, but I would also follow up with your school counselor. It’s been a couple of years, but I remember there were forms that my S, we as parents, and his counselor all had to sign. Midd’s admissions office isn’t so big that oversights like that can’t be corrected. Best of luck to you!</p>
