extended time for ACT and teachers' letter

I am planning to request extended time for my younger kid, she has depression and anxiety disorder and ADD. In 9th grade was admitted to a mental hospital twice, but the 504 plan did not come until the 2nd half of 10th grade where her grades went to Ds and Es. After the extended time, her grades went back up to As and Bs, I don’t have a neuropsych report, and only the doctor’s notes, the 504 plan and her grades, in the case of college board, they provided a document for the teachers to write, but ACT does not provide similar form, has anyone used the college board form for ACT ? (BTW, I knew about the “required” documents link from ACT and SAT, but I am hoping I do not have to go through the process a neuropsych eval) If I cannot use the form from college board, what type of letter coming from the teachers will be helpful? - thanks

There is a teacher survey form for the ACT. Just search “Teacher Survey Form-ACT” and the .pdf should be at the top of the list. I have filled about three of these forms out over the years for my students; the last one I filled out was last summer. So, unless they’ve changed the policy since last year…

D’s teachers filled out forms for the CB, but none for the ACT. Can’t comment about the neuropysch eval; D’s issues revolve around physical disability. I believe that one of the most important things is that there is a 504/school accommodations plan in place.

My kid went to a school that does not have 504s (private), and she got ACT accommodations. But the school did provide documentation on accommodations she had been granted and was actively using (and we had dr reports going back before HS, too). So just saying a 504 is not an actual requirement.

@intparent Likewise for my kiddo – that’s why I mentioned “school accommodations plan” in addition/in lieu of 504 plans, but I should have made it more clear. The important thing, as you mentioned, the student needs to have an accommodations in place at school.
