Failure to disclose previous doctoral programs

I know that undergrad applicants are required to disclose other schools they have attended, and provide transcripts- and that there is a database for admissions people to check.

Is this true at the grad/doctoral level?

Can someone send me a link on this, if you know? I found one but I would like a succinct, clear policy on this and what the consequences for not disclosing.

Doesn’t National Student Clearinghouse include graduate level enrollment and degree checking in its services? Its web site does not say that it is limited to undergraduate level enrollment and degree checking when it comes to college enrollment and degree checking.

The Clearinghouse is for “postsecondary” which presumably includes doctoral programs, yes.
