Harvard Common App Teacher Supplement

What would a typical teacher supplement for Harvard look like so it's not over-exaggerated? What i'm specificially looking for is the answers for this:

Compared to other students in his or her class year, how do you rate this student in terms of:

Area Below Average Average Good (above average) Very good (well above average) Excellent (top 10%) Outstanding (top 5%) One of the top few I've encountered

Academic Achievement
Intellectual promise
Quality of Writing
Creative, original thought
Productive class discussion
Respect accorded by faculty
Disciplined work habits
Maturity
Motivation
Leadership
Integrity
Reaction to setbacks
Concern for others
Self-confidence
Initiative, independence
Overall

Thanks!

Also examples of:
How long have you known this student and in what context?
What are the first words that come to your mind to describe this student?

^^ Why do you ask? Are you writing your own rec?

Give the form to your teacher. If they don’t know what to write, send them to MIT’s website for teachers recs: [Writing Recommendations | MIT Admissions](How to write good letters of recommendation | MIT Admissions)

BTW: Teacher’s have known you for as long as you have known them – one semester, one year, two years (in a classroom situation). I would imagine that all selective colleges look for students who are in the top 5%-10%.

nope, i’m not. that would be great though)
