help with my supplementary recommendation?

over the summer, i took a math class at a college summer program. i really enjoyed the class and did well in it (there were other college students in the class; i'm a high school senior) so i asked the teacher for a recommendation.

my question is, should i put his recommendation on the commonapp as a teacher recommendation? or a supplementary recc. I thought the supplement would be fine, but under the drop menu, the options are more along the lines of 'coach, boss, arts teacher, etc'. so there isn't an option for teacher in the supplement menu. i ended up putting him under "other", but i'm not sure if that's the right move.

along that line, around 3/4 of the colleges I'm applying to only allow for 2 teacher recs, and i already have 2 from school, so i would probably need to list him as a supplement anyway for those, at least.

and one last thing: i'm applying to yale early action, but they only allow for 2 teacher recs and no supplements on the commonapp. i checked online and found that they allow supplements, but only if i think it would really affect my recommendation. it also said that supplements can often hurt the applicant if they aren't that useful. while my summer math teacher probably wouldn't write a hugely different rec. than my high school math teacher, i think my summer teacher captured more of my interest in learning, as i basically answered questions more than anyone in the class, and went to his extra credit sessions (i thought the class was more interesting than my high school math). so should i ignore yale for my supplement?

thanks!
