<p>The subject was brought up during orientation. I am wondering is this is something we should do and who do we give it to? I am assuming the Student Health Center and my insurance company. What if he ends up in the hospital for some reason, do I just keep a copy at home and provide it as needed?</p>

<p>If your student wants you to have access to his academic records and financial account, then you need to have releases completed and submitted. I think this can be done online. </p>
<p>Otherwise, if he’s trreated at the Wexner Medical Center you might need another release form. Probably it would be best to contact them and ask what they’d want.</p>
<p>NervousNellie did you ever get an answer in this? I’ll be meeting wih a doctor in the Wilce center on Tues while at orientation, so I intend to ask him then. Let me know if you have any other outstanding questions about release forms, etc., I’m happy to ask doc while I have his time and post on CC or send you a PM.</p>
<p>@ Nova88-yes-if you could find out more about that I would be grateful. I am still not sure how all of that works. I am listed as S emergency contact, but I am sure I will needs some sort of medical information release. From what I understand HIPAA is the common one and I would imagine I will just keep a copy with me. As far as the Wilce Center, I saw something on their website-they may have an additional form.</p>
<p>Cannot believe they move in 6 weeks from tomorrow. Not sure I am ready for this. S has been having a very relaxing summer. Hope he is prepared to get back to work! :-)</p>

<p>Excited about orientation. S is eager to meet some kids he will know when he arrives in August S has to be on campus August 13th for R-Lead. He is looking forward to it. Reality is sinking in, college is a wonderful experience, AND hard work!! They’ll get it soon enough! It’s all good! :)</p>
