How to List Colleges and Universities Attended in the UC TAG Application?

<p>So I went to a private university from 8/2012 to 4/2014 (I withdrew in the spring semester in February). I went to a CCC in the summer of 2013. After I withdrew from the private university, I enrolled at the same CCC last summer. Should I have 3 separate entries for my "Colleges and Universities Attended"? </p>
