is it means to send the letter back?

<p>in the letter sent to me after I apply for UC San Diego, they said my phone number was missing and asked me to add this imformation.
"please enter the requested information in the appropriate spaces and return this form by 12/23/08"
Does it mean that I should write my phone number in the space behind the printed "phone number" at the top of this paper?
or, should I add my phone number by the internet?</p>

<p>Also, it seems that I need to provide my score report in Jan 09? but, what about I can only get my transcript in March? Since the universities in Japan start their spring semester at April.</p>

<p>I would contact the email address from the confirmation page of the application and ask, most here are not aware of the policies for international transfers.
