You are in-state.
I have your UC GPA at 3.26, below the UCSC average admit of 3.56 or so.
I have your SAT I at 1620, slightly below UCSC's average of 1680, but well within the middle 50% which is 1530 to 1890.
Your SAT IIs are fairly poor at 1030.

You will get extra points for your low income status
You will get extra points for your 1st generation college status
You will get extra points for your parent being disabled
You will get extra points for a particularly challenging courseload
Your ECs are excellent considering the family situation.
They probably will give you a slight break for two grandparents having passed away in the same year

You will not get extra points for having a single parent working

Based upon all of this, you will probably be accepted, but it's not a guaranteed thing. I'd try to write some good essays taking about your family from a first-person point of view--and also talking about what you hope to accomplish in your career ahead (tie this in with the major you choose).

Good luck.
