Personal Recommendation Letter for Philips Academy

Hello,
I am trying to apply to Phillips Academy, and it says they require a personal recommendation. I found a PDF on Gateway to Prep Schools, and it’s just a sheet of paper with a few questions.
I was wondering what the recommendation letter has to look like? Does it have to be an actual letter, or do I just send in the papers with the questions that I got on gateway prep?

Thank you if you have answers to this question, no one in my family has ever applied to Phillips Academy, so I’m a little lost.

Recommendations are blind. My recollection is that you will provide the email addresses of your references to Gateway, and the recommendation forms are sent directly through Gateway. The application season for the students starting in the Fall is, however, already complete–they were due Feb 1st–and the essays for the class starting in Fall 2019 are not yet available.
