question about what to put down for father

<p>I'm a junior and applying for the summer prep program. I have never had to fill out one of these type of applications that asks these personal questions, etc so I don't know what to put down for my situation. </p>

<p>I live with my mom and I'm not entirely sure what to put down for my father. I know my fathers name and we have the same last name. My parents were never married. But he's always been in and out of my life. Last time I checked he was working at a fast food place. I don't know where he lives and whatnot. I don't have a way to give them his income (papers or otherwise) and I'm pretty sure he hasn't kept up with his taxes. I don't know what to put down for him. </p>

<p>Also, I have 5 siblings. My brother lives with me and we're both juniors (though not twins) and then the other 4 are WAY older than us and have their own lives in different states and are half-siblings. Do I still include them even if they don't live with us?</p>

<p>Our family of 3 had less than $25k income last year</p>

<p>I would fill in everything that you can. They have little boxes where you can enter information and explain your situation.</p>

<p>i dont think you would have to add the older siblings (but you should call and ask). as for your father, just fill out everything you can.</p>
