What if your school doesn't have clubs or varsity teams?

So I’m a US citizen in Germany applying to Northwestern and Stanford and looking on this website made me realize that my school really doesn’t have any clubs. My extracurriculars include peer mediation, volunteering at a hospital, german interpretation as a job, and lots of tutoring but I’m pretty sure that’s not enough for someplace like Stanford. Will these colleges know that my school has limited EC opportunities?

School clubs aren’t usually enough for Stanford, either. They look for unusual, in-depth ECs. Your issue isn’t really lack of school clubs.

It does not matter one bit if your activities are in school or out of school.

I think your ECs are good. You can’t lie, so go with what you have. ECs won’t be a deciding factor. Far. More I portant to have good recommendations from teachers and counselors.
