I have one excellent recommendation, but have one mediocre one. However, there is one teacher who taught me in tenth grade who knows me very well. She was also my yearbook advisor, and knows me personally. Should I send an optional recommendation from her, or is one excellent and one mediocre one enough?

i'd definitely send the optional recommendation... i sent one from my CSF advisor, the club that i'm president of, and since we were really cool on personal terms, i let her know which parts of our relationship i was hoping she could elaborate on... if you're really cool with your yearbook advisor, then let her know to put lesser focus on the schooling part (since the teachers will be doing that) and emphasize her personal experience with you doing the yearbook... the total package of all the recommendations will sound good that way

Yes yes yes, this is what the optional one is for..
