AP Score Cancellation question!?

I am filling out the form right now to cancel an ap test i took and it asks for the “exam code”…how do i figure out what the code is? Also I don’t know my school code either…

Oh and I don’t know what the “AP Number associated with exam” is either…

AP number associated with exam is your personal AP number you used to take the test. Exam code is found on a list below the cancellation form.
