arts supplement?

<p>I found the following information from another post:
"Slides of your art work may be submitted as a supplement to your application for admission to Yale College. All materials must be sent to the Office of Undergraduate Admissions by the appropriate application Deadline. Submit no more than one sheet of 20 color transparencies of 35 mm size. These transparencies should be enclosed in a standard 8 1/2 x 11 plastic slide sheet designed for this purpose. The slides should specifically include some drawing samples. Choose pieces of your work which demonstrate the fullest possible range of your talents. Try to ensure that your technical and expressive abilities are equally represented. A simple typewritten inventory must accompany each plastic sheet. The inventory should include your full name (as it appears on your admissions application), your U.S. social security number (if available), your high school, and the state or country in which your high school is located.
Each slide must be numbered and the corresponding inventory entry should include the full name, medium, size, and date of the work. Please indicate if the work was done from life. Your supplementary material will not be returned. Please do not send us originals.

<p>However, do we need to submit the common app. arts supplement for with this? Do we also include a resume and recommendation?</p>

<p>My son just sent the slides with inventory sheet, just like the way they explained above.</p>

<p>Oh, and another thing... I've noticed that they say they won't review any videotapes or DVDs. But what if I burn a video onto a CD?
Would they consider that?</p>

<p>My D sent slides of her work last year. This was done on a plastic sleeve that has slots for slides and can be bought at a photo supplies shop. You must also make an inventory of each slide, numbering, titling, and stating the medium used to create the piece. We took them at their word, and since they said slides, and not a CD or DVD, we sent slides. I would recommend that you do exactly what they ask. They are Yale, after all! Also, I don't think she sent the arts supplement part of the common app to Yale. I know she sent it to one of the schools. Ask Yale admissions that question. Good luck!</p>
