When asking for one do you just go up and ask if they can write you a recommendation? Or is there some way of going about?

I printed out any rec. sheets provided by the schools/scholarship companies.

I also made sure I provided anyone I wanted a rec. from with an addressed envelope and stamp. They are doing you a favor by writing the rec. for you. They shouldn't have to shell out money for a stamp.

It's also good to provide the writer of the rec with some info about the school/scholarship you are applying to, so they know what to focus on when writing it.

And remember, send out thank you notes, and inform the writer of the rec of the decision when it is made.

Oh! And remember to say please and thank you.

My counselor provided me with a sheet in which I listed all my activities in such. Do I ask them if they could write a rec. for me, then follow that with the sheet?

If the teachers you are asking are unfamiliar with some of your activities/achievements, then by all means make a copy of the sheet and include it. The more information they have, the better your rec. will be (in my opinion).
