<p>I looked at the app last year, and there is a form for teachers to fill out. It's similar to the CommonApp's teacher form in the questions asked and the checklist of qualities. Teachers can write a letter and attach it to the form, if they don't want to write the statement directly onto the form.</p>

<p>Well, when the c/o 2012 application comes out (which should be any day now), it'll be one of the forms on there (forms 4a and 4b, I think).</p>
