I received a C first semester and a C+ second semester in Calc BC junior year, and then received a 5 on the AP test. Aside from a B in AP Physics, this is the only non-A on my transcript.

How should I approach my teacher to get the grade changed to at least a B-? I haven't heard of him changing grades and I've heard he denies requests from B+ to A- based on a 5 on the AP test, but from C+ to B- is quite a different case, I think.

I would think final grades are final. Since AP scores aren’t released until July, your final term grades are probably out before then. I highly doubt teachers would give grade boosts for good AP scores…

I guess I should add that AP Chemistry teachers at my high school change final grades for sophomores and juniors that earn a 5 on the AP test but do not receive an A in the class. I think that is the policy for juniors in AP Biology as well.

Each high school teacher usually has complete control over his grading and grade-changing policies. Unless you have it in writing e.g. on the course syllabus at the beginning of the academic yr, probably out of luck.

Colleges will know from your 5 on the AP that you have mastery of Calculus & likely your high school teacher grades tough for that subject.
