Can i get into a good college with my LD?

Well i actually don't know if this really is an LD, but here goes.
Grades:
freshman year: 74
Sophomore: 79
Junior year: (so far, and improving) 88

I started taking medication for my ADD towards the end of my freshman year, which is why the upward trend begins there. However, I had this weird reading problem, that actually came from having lymes disease when i was really young. I wasn't diagnosed with lyme until long after i had it, which cause some long term problems, one of them being this reading thing. I really don't know what it was called, or even much about it. However, while i was being tested i remember the doctor being surprised that i was even pulling off C's. (Also, i go to the #2 ranked high school in CT) Anyways, I needed to go to see the eye doctor three times per week, for all of sophomore year in order to correct my vision. After all of that, i was finally able to perform well in school. I'm very mad because my gpa is not going to be able to get me into any good college, even though i know i have the ability. When i took the psats, i scored in the 90 percentile for my school, and our school is one of the best anywhere when it comes to standardized testing. Will colleges recognize this problem when i start applying? Do I have any chance at getting into a good college?
p.s. - Is this the right forum to be posting this in?
Thank You!

Just mention it when applying to colleges, colleges love to see an upward trend in general, also junior year is the most important which your acceling at. Whether or not you can get into a “good” college depends on:

-What is a good college to you?

-Extra curriculars
