I think what this student needs to clearly understand is the difference between a tradition Pre-med route, and the BS/MD and BS/DO programs.

Assuming your ECs are not embellished, they are extraordinary – And don’t be afraid of bragging about them. The goal of the application for competitive schools is to stand out. One wouldn’t hold back their GPA or SAT score for being too good, same with ECs.
Those are the types of things that can make you stand out from the other high academic achievers. Just make sure to be honest without embellishment. If the AO googles your non-profit or the news media about you, it better match whatever you put in the application. Nothing wrong with bragging in an application, as long as it’s true. (and there is still a way to write it up without sounding obnoxious).

As others have said, BS/MD programs are ultracompetitive. You will be a serious candidate. You are a match for many of the schools on your list in terms of their regular Bio/pre-med programs. Despite being a great candidate, you’ll still need to keep fingers crossed for the BS/MD combined programs. How are you STEM AP scores?

Many of these programs require maintaining a high college GPA and/or getting a high MCAT score.

Actually, in most BS/MD programs, the requirements are somewhat relaxed than for a student entering the same medical school through the traditional route.
GPA requirements for some colleges are 3.2, 3.5, 3.65, or 3.75.
And MCAT requirements are 66th percentile to 90th percentile.
