Chances for low gpa and high test scores (please help)

I think this is a rather unique situation, as I haven’t seen any similar threads. For context, I go to a very competetive public high school

Stats:
Unweighted GPA- 3.5 (I can get it up to a 3.6 by the end of sem 1 senior year)
Weighted- 4.7
Freshman and first semester of sophomore year it was mostly B’s and some A’s. 2nd semester of soph year I had 1 B and the rest were A’s. 1st semester of junior year was all A’s, but then 2nd semester of junior year dipped and I got 4 B’s and 2 A’s.
I’ve taken 10 AP’s so far, 4s and 5s on all of them, and I’m taking 5 more next year. I also took a multivariable calculus course at a university junior year. The rest of my classes are all honors.

36 composite on the ACT (35 in reading, rest were 36’s)
SAT Subject Tests- Math II(800), Chemistry (800), Physics (790)

EC’s:
Debate (leadership position)
HOSA, a competitive medical club (multiple awards at state)
Red Cross Club (leadership position)
I’m heavily involved in a well known non-profit, where I’ve carried out my own large scale projects

I don’t have any notable awards other than from ECs. I’m doing a couple internships over the summer that are related to what I want to do (biotech or biomed engineering)

What do you think my chances would be for schools like Northwestern, UChicago, Case Western, and UC Berkeley? What are the best colleges you think I can get into with my stats?
Also, would mentioning that I have ADHD help or hurt my chances for admission (I was diagnosed junior year)?
