Common App teacher recommendation form - HELP

<p>I'm really confused with the common app teacher recommendation process.</p>

<p>Where can I get the Common Application Teacher Recommendation Form?</p>

<p>When my teacher gets assigned as a recommender, is she provided with the form online?
Is all she has to do complete the form online and then submit her recommendation letter?</p>

<p>You don’t. You just send invitation to the teacher(s) on CommonApp and they will receive the instruction to login and submit the LOR.
After you invite the teacher, you may selectively assign him/her to each school you are going to apply. Whatever they uploaded will be linked to your application to that school for downloading.</p>
