<p>what do you mean by counselor recommendation? I think you can call Sec. School report a counselor recommendation because there is no other separate form for counselor evaluations. All the evaluation checkmarks are included on Sec. school report form. HOWEVER, In any case, It can NOT go there by itself. Transcript is necessary so that colleges see all your grades and classes.</p>
<p>Total, you have to have sent:
1. Sec. school report,( which includes counselor letter or a "summary statement" as they call it),
2.transcript,
3.explanation paper on grading and GPA weighting policies of that particular school.</p>
<p>Did she send all that together with whatever she submitted? If yes, then you are fine.</p>

<p>Well if she filled out some form online and she said it was a form for a counselor (or any other school official) then I guess it was a secondary school report. I can not recall any separate recommendation form for a counselor. I know teachers have separate ones, but the counselors put their evaluations on the Secondary school report.</p>
<p>And No, you dont have to send another paper version of the form. That will confuse things more than they are already. Although it is always better to send those papers either All by mail, or All online, I think they will be able to match the papers in your case. So.. dont worry.</p>
