Credit Card declined by collegeboard (HELP!!!)

<p>I was waitlisted and took the November SATs. A few days ago, I got an email from college board saying that they declined my card and I had 3 options for payment.</p>

<p>How do I pay them by fax? They gave me a form to fill and fax them, so:</p>

<p>Do I have to PRINT the form, FILL it, SCAN it, then FAX it?</p>
