Do US universities receive our mock exam result if they are after our Mid-Year Report?

I thought US universities only received the Mid Year Report after the submission of the application, and then the final results from an official examination. However, my school’s teacher sent me this:

  1.   Your Term 3 grades, basically your Mock Exam results, go on your school transcript,

  2.   your transcript MUST be sent to the uni that you decide to attend

  3.   ALL US offers are “conditional” 

  4.   the main condition is no “significant” drop from your mid-year grades

  5.   they can withdraw their previous acceptance 

  6.   they may not listen to your explanations for why your T3 grades/Mocks have dropped and your promises that your “real” exams will be better.

Is this true?
