Does Washington University require a separate form for artistic supplement?

I was just wondering if Wash U. has a separate form for artisitic supplement. Or is it the common app artistic supplement form.

there is no particular form you need to use -
i don’t remember what i did; but you can use the common app form
just make sure your info is on EVERYTHING you’re sending.

If they do, it’s not part of their suppliment to the Common App. Email the admissions office if you want to add something like that to your application.

I emailed them and this is what they said:

"Hello,

Thank you for your interest in Washington University. I’m happy to answer your question.

We are happy to receive and review any supplemental materials you wish to provide. Even if you are not applying to a school that requires it, we are happy to receive a portfolio and/or the common application arts supplement! Just follow the same format requirements as the ones for the Sam Fox School for Design and Visual Arts. Don’t forget to label it with your name and ID number!"

Since it’s not part of their supplement on the Common App, just follow the Sat Fox School’s guidelines in submitting a portfolio.
