<p>Newly-accepted students will receive a Pre-Matriculation Health Record by mail that outlines medical clearance requirements for entering students. Both you and a healthcare provider must sign this form. Because the Pre-Matriculation Health Record will be scanned, it cannot be posted online.</p>

<p>The deadline by which we must receive your medical clearance compliance documentation varies by semester. It is printed on the letter accompanying the Pre-Matriculation Health Record.</p>

<p>If you have lost your form, call our Immunization Compliance department at (512) 475-8301 to request another copy.</p>

<p>I just got mine in the mail two days ago... I'm guessing they just sent them out, so you should probably receive yours soon.</p>
