<p>Can't you leave parts 1,2 and 5 blank and just print the packet. Or just get the dates before you print out the forms. Yeah, it shouldn't be hard.</p>
<p>Yeah, they just re-print the information from other pages in the final packet. I am sure you can fill it out using a pen afterwards on the final packet that your doctor needs to complete. I am guessing that you can get the immunization dates then.</p>
<p>call your Dr.'s office and get the dates or get them from your mom who probably has a list from all the other times your needed them like when you applied to college that you are transferring from...(poor grammar. sorry) but the form DOES NOT rpint out write until you fill in the required info -</p>
<p>This is something that you should consult your parents about.</p>
<p>They have needed your immunization history at various times in your life (you couldn't have gone to school without it). They should know how you can obtain it.</p>
<p>If you have consistently used one primary care physician for at least a few years, that physician's records probably include your full immunization history. If not, things may be a little more complicated, but your parents have dealt with this before.</p>
<p>my doctor (the one i go to occasionally and not really a family doctor)...did not have my records, but my high school had a copy of my shots (b/c as soon as i got one they wanted record of it)...or i think your county's health dept has record of it... </p>
<p>as for the meningitis vaccine...that again is optional but families who are overprotective or have the $ to pay for it will usually do it...</p>
<p>the cornell student insurance is kind of expensive $1,500 but i guess its a good policy and never hurts to be safe...make sure you go to gannet to make the most of it!</p>

<p>most people just tend to get the one that allows you to use the fitness center (gym)...girls will tend to get that + the one that allows you to take the fitness classes (aerobic, yoga, or whatever new dance fitness thing is current)....</p>
