filling out teacher rec form

<p>u know on the teacher rec form, there's a part where the student has to fill in?</p>

<p>what about on the 2nd page?</p>

<p>it's written in sideways and we have to fill out application #, social #, and name
do we still have to fill these out?
if so, what is the application ID code?</p>

<p>i filled it in on the 2nd page. i called the admissions and asked if i needed to put an application id code and they said i didn't need one. if you got an application packet via mail then you should have an app ID code and should put it on there.</p>
