The profile is a page your school does up describing itself so that colleges know what the academic environment of your campus is. It's usually one page. Ours describes the campus, including the high number of NMSFs, AP offerings, etc.

Talk to your guidance counselor. Unless your school is very small, I doubt that you're the first student in your school to apply to a US school.

Ask your guidance counselor what your school usually sends with transcripts? Even if they don't call it a "school profile" they most likely send some sort of document. Here's some info on what they usually include. High School Profiles - Ask The Dean

FWIW I think I wouldn't bother with the assessment and planning part. Maybe you ought to sit down with your GC and show her what's usually included so she can prepare something. I think the idea is to give some context to the transcript (Are advanced or honors courses available? Did you take challenging classes? Is there an explanation of your grading system--how grades are weighed-- and how class rank is determined?) If that's on the transcript itself that's fine. If it's not maybe your counselor can address those issues in her rec. Good luck.
