<p>MyAdmissions status for the Hepatitis B immunization is “Additional Documentation Required.” I’ve faxed all the required documents in a while ago, and then I faxed them again recently just in case. The “Cowell Student Health Center at: 530-754-9596” which I was told to contact doesn’t have a live representative. It’s also unclear when is the exact deadline for submitting the documentation. What should I do?</p>
<p>i just did mine recently… and all it asked for were the dates you got the shots, and your clinic’s information. you enter it on the web, i didnt have to fax in anything for hep b .</p>
<p>I guess this doesn’t apply to everyone, but my account says:</p>
<p>“You have a hold on your campus account as a result of a random audit being conducted by Cowell Student Health Center. You are required to provide documentation supporting the information on the Hepatitis B Immunity Statement that you submitted previously at this web site. A hold has been placed on your campus account pending receipt of that documentation. Audit notification and compliance information was sent to your UC Davis email account.”</p>
<p>so I do need to fax stuff.</p>
