home schooling and LD testing

How does LD and ADHD testing work when you are home schooled? My elementary school aged nephews and niece were recently transitioned out of traditional school into home school, one is already diagnosed with high functioning autism and ADHD, and my sister is considering having another evaluated for ADHD. She is specifically wondering about the teacher report that is typically requested, given that she is the only teacher her children have at the moment. She says she does not think her doctor would be willing to diagnose without the teacher evaluation and she isn't sure if she could fill it out being the parent as well.

And how will accommodation and documentation sort of issues work when it comes time to take the ACT and go to college? I was not home schooled and I know I was unable to get accommodations for the ACT in large part because I had been recently diagnosed and I did not have a history of accommodation documented nor did I even have an IEP in place yet at the time. I would imagine my nephews would not have these things either given their circumstances but if that's the case then how does this work for them?
