How do I request an application for Northwestern’s PLME (BS/MD) program?

I’m aware of what to include in the request for application, I just don’t know how to request it.

First thing is to know that it is not called PLME at Northwestern - that is Brown’s program. At Northwestern, it is HPME. And there is info on the NU application page that essentially says stay tuned: https://admissions.northwestern.edu/apply/requirements.html
