How does one go about doing this? When people say "I studied for 5 hours before the day of the SAT", what do they really mean? Did they look at/memorize vocab, study grammar/math rules, or just do practice tests? I mean, it's not like you can actually prep for the content of the test since nobody will really know what is on it, unlike a school chemistry test, for example.

Depending on what score you want to get, you need to memorize hundreds of words, know the 4 important math rules, and most importantly, change the way you take the test. To do this, take a practice test. YOU NEED to take practice tests, it's pointless to take an SAT without even taking one practice test.
