I interned for a campaign from April-June this year and I got a letter of recommendation from my supervisor. However, I lost contact of her so I don't know how to send in my letter, as she is supposed to be the one to send it in for me.

Don't tell me I need to have my recommender send it in for me; I already called the college I'm applying to and they said that it's fine if I turned it in myself. However... how exactly do I mail it in?

Like do I just put the recommendations into a yellow envelope and mail it to the admissions office? What do I write on the envelope so that the college knows that inside the yellow envelope are my letters of rec. and how will they know that it's for me specifically? I'm sure there are other people with the same name as me, so would I put my D.O.B. and what not? Please let me know, thanks!
