<p>I jus got my I20 form from USC today but it does not include my middle name. I hold a Canadian passport and theres only a GIVEN NAMES section where you put all your names other than the SURNAME. However, I use my middle name in all of the testing I did and it's also on my high school diploma. I was wondering if missing the middle name would be a problem for the F1 visa? </p>

<p>I haven’t received mine yet. I emailed USC and they told me they were still processing them. It takes them about 3 months after you got accepted to do so, could be 4. So we should be getting it in the next month.</p>
<p>Hey, so I’m sorry to revive this, but are we supposed to make a formal request to receive the I-20 form? The rest of the schools I was accepted to sent the form in with my acceptance, but I haven’t received it from USC.</p>
<p>I realize that there is a request form I can print online, but the details requirements seem to imply that it is for people who have already received an I-20 form and require a new one.</p>
<p>I did look through the forms but as I’d mentioned earlier, the little details on the I-20 request form implied that it was for people who needed a NEW I-20 form. It seems the implications are indeed true:</p>
