This is kind of urgent so although I know this may not be completely appropriate to post, I’d really appreciate an answer.
I was filling my e-health thing and it asks for my doses. On my sheet I sometimes had gotten the vaccine twice on different dates but it is considered one dose. Which one do I enter? The most recent one?
Also, do I need to ever show at least the copy of the actual sheet with the stamps? So far I’ve only read that I needed to fill the forms online.
Is it for UCLA? Those are only recommended, not required.

I’d put the most recent dose if it is due immediately. If not, call the Ashe center in the morning so they can assist you.
